Your Role in a Nutshell:
As a Bilingual Care Coordinator, you will provide monthly care coordination services to a panel of patients, ensuring they receive ongoing support between healthcare visits. You will assist patients with managing chronic conditions by educating them on their health, working with providers to adjust treatment plans, and coordinating care with different teams. This role requires clear communication, empathy, and the ability to prioritize patient needs in a remote setting.
You will conduct phone calls with patients, their families, and healthcare providers, developing customized care plans to ensure the patients receive the best possible care. Each day, you'll engage in phone interactions to deliver your care coordination services, maintaining high productivity while focusing on the individual needs of each patient.
Key Responsibilities:
Conduct monthly care coordination calls with patients, medical offices, and pharmacies, spending 7-8 hours daily on the phone.
Create and revise personalized care plans for each patient, working closely with providers.
Identify social determinants of health, gaps in care, and eligibility for additional services, coordinating with other healthcare resources as needed.
Empower patients to manage their health through the implementation of actionable care plans.
Make 30-50 calls per day to achieve meaningful interactions and provide 20 minutes of care coordination service each month.
Maintain strict adherence to confidentiality, legal, ethical, and accreditation standards.
Qualifications:
At least 1 year of experience as a Certified/Registered Medical Assistant.
Fluency in both Spanish and English, with excellent verbal and written communication skills.
Familiarity with multiple EMR/EHR systems and proficiency with Microsoft Office and Google Suite. Ability to quickly learn new software.
Highly professional phone demeanor with a compassionate and engaging tone.
Strong critical thinking skills to respond to live patient inquiries and address needs.
Ability to work independently in a remote setting, managing time and priorities effectively.
Call center and/or triage experience is a plus.
Experience working with a geriatric population or in case management/care coordination is a plus.
Working Conditions:
You will work a 40-hour schedule, Monday through Friday.
A private home office setup is required, with a second monitor and fast, reliable internet (at least 50 mbps upload speed).
