Community Care Coordinator / Social Worker

This something-new-every-day job is for someone who wants to make a difference in the lives of veterans. As a Care Coordinator II with the Veteran’s Count Program, you will provide community-based case management and social work services that will establish a positive impact on veterans, service members and their families. You will develop rapport and engage in a holistic client-driven plan for stability, facilitate connection to long-term benefits and services that are deserved and earned. In this role, you will be working primarily in a community-based setting to provide guidance and assistance with the development of skills and access to resources that will help address substance misuse, obtaining health, mental health, and recovery services, housing insecurity, veteran benefits, financial sustainability, such as money management, and applying for SSDI/SSI.

Responsibilities include, but are not limited to:

  • Providing community-based services, assessments, consultations, referrals, and support in a variety of settings.
  • Complete treatment notes, treatment plans, and assessments in the electronic medical record (EMR) system.
  • Collaborate with various programs, providers, and stakeholders to best provide streamlined support, guidance, and assistance to at-risk individuals in the community.
  • Effectively navigate crisis-level situations and provide beneficial support & resources as needed.
Skills needed to be successful in this role:

  • Ability to meet clients ‘where they are at’ and develop rapport despite presenting challenges
  • Attention to detail and ability to navigate databases and systems to retrieve and enter critical data to ensure your veteran clients are enrolled in all deserved programs
  • Quick response time, ability to prioritize on the fly, and flexibility in how & when services are delivered.
  • Confidence working independently, using sound judgment on your need for consultation and guidance
  • Effectively determine the needs of your veteran clients and balance home-based and community-based service delivery
What you’ll need:

  • Master’s degree in human services or related field (counseling, social work, social & behavioral sciences, education, human development, mental health, psychology, rehabilitation services, and sociology) with two (2)+ years relevant experience in a community-based case management role (or similar).
  • OR
  • Bachelor’s degree with at least five years related experience in a community-based case management role (or similar).
  • Experience in navigating and coordinating benefits / services preferred.
  • Understanding of military structure and culture preferred.
  • Valid drivers license and reliable transportation
Hours: Full time, 40 hours per week. Work will be performed in the community, from home, and within the local office (Manchester).

Compensation: $26.10 - $30.60 per hour, depending on qualifications, experience, and credentials
